We have provided 2 convenient options to complete your new patient paperwork.

Select either Online Forms to complete and submit entirely online or Printable Forms where you print, complete the forms and bring them with you at the time of your appointment.

Please give us a call at 713-890-7595 once you have submitted your Online Form.

Online Forms

• New Patient Form – if you’re a completely new patient to the practice.
• Returning Patient Form – for returning patients to update their contact information and medical history.
• Emergency Office Visit Only – for new patients who require an emergency office visit.
• Optical Visit Only – for new patients who have valid outside prescriptions and are looking to purchase frames and/or lenses.
